COMMUNITY HEALTH ASSESSMENT, DIAGNOSIS, AND CARE PLAN
IntroductionA community health assessment for a neighborhood in New York City was performed as a part of this research. In this research, the use of the terms community and neighborhood is interchangeable. A nursing diagnosis and a nursing care plan for the community were also a part of this research.
The community health assessment was performed within the framework of the Community Health Assessment Tool (CHAT). The assessment is presented in the format of the 11 sections of the tool. The community assessed was District 6 in the Borough of Manhattan, which lies between the East River and Park Avenue, between 14th and 34th Streets. Additionally, the community population for this assessment was defined as the population segment aged 65 years old and older.
I. Health Perception Health Management Pattern
A. Protective services: Police services are provided to residents of the neighborhood through two NYPD precinct stations located within the neighborhood. The major types of crime affecting elderly population group in the neighborhood are burglary and larceny. Fire services are provided to the community through a NYFD precinct station located within the community. A disaster control plan for the community exists and is coordinated by the municipal department responsible for civil defense. All emergency and medical service agencies located within the community have responsibilities under the disaster control plan. B. Morbidity and mortality statistics: Diseases most frequently affecting the elderly population within the neighborhood are heart and circulatory problems, cancers, diabetes, pneumonia, and arthritis. The leading cause of death among this population group are the diseases of the heart and circulatory group. The crude death rate of 9.7 per one hundred thousand persons.
