COMMUNITY HEALTH ASSESSMENT, DIAGNOSIS, AND CARE PLAN

IntroductionA community health assessment for a neighborhood in New York City was performed as a part of this research. In this research, the use of the terms community and neighborhood is interchangeable. A nursing diagnosis and a nursing care plan for the community were also a part of this research.

The community health assessment was performed within the framework of the Community Health Assessment Tool (CHAT). The assessment is presented in the format of the 11 sections of the tool. The community assessed was District 6 in the Borough of Manhattan, which lies between the East River and Park Avenue, between 14th and 34th Streets. Additionally, the community population for this assessment was defined as the population segment aged 65 years old and older.

I. HealthPerceptionHealthManagement Pattern

A. Protective services: Police services are provided to residents of the neighborhood through two NYPD precinct stations located within the neighborhood. The major types of crime affecting elderly population group in the neighborhood are burglary and larceny. Fire services are provided to the community through a NYFD precinct station located within the community. A disaster control plan for the community exists and is coordinated by the municipal department responsible for civil defense. ghborhood. B. Recreation and fitness: Several city parks and one private park are available to residents in the neighborhood. Recreation and fitness programs are conducted by senior citizens service centers operated by the municipal government. V. SleepRest Pattern A. Business: Most businesses located within the community operate during traditional day light hours. Some grocery and drug stores operate extended hours. Generally, however, the hours of business operation within the community are not a community health concern. B. Industry: Construction activity appears to be endemic within the community. Most construction projects cease operations during hours of darkness; however, work continues on some projects at night with resulting interference with the sleep patterns of community residents. VI. CognitivePerceptual Pattern A. Government: The various department of the municipal government of the City of New York operate the municipal agencies located within the community. The Manhattan Borough office acts on behalf of all Manhattan residents, including those within the community, in dealings with the municipal government.
