Assent for Participation in the Psychosocial Assessment and Treatment Project

You are being asked to give permission for your child to participate in a research study. The purpose of the study is to conduct a psychosocial assessment of your child by studying and observing his/her behavior and interactions with the family in the home setting in order to formulate an appropriate treatment plan. The expected duration of this project is two one-hour weekly sessions. Based on the observations, a follow-up treatment plan may need to be designed and implemented, which will require the further involvement of the child in the future. This procedure is unlikely to cause stress, pain or any other unpleasant reactions to the child involved.

Since your child will simply be observed and studied without direct interaction, participation in this project will have minimal risks. Nonetheless, the child and parents will likely benefit from the findings of this assessment that will enable them to learn about the status of the child's psychosocial development. Furthermore, if needed, the child will also be able to receive further treatment interventions. If you have any concerns about the risks or benefits of participating in this study, you can contact David Glassman or the IRB office at the numbers indicated above.

Costs and Payments to the Participants:

There are no costs to you or payments made for participating in this study.

All information obtained in this study is strictly confidential unless disclosure is required by law. Your child will be assigned a study number, instead of his/her name, in the research field note
