A. The health care promotion activity will be to thoroughly educate patients with diabetes about the importance of foot care, and the role of the control of diabetes in this important aspect of the care and management of diabetes. The only way to maintain healthy feet in patients with diabetes is to thoroughly educate them in why it is important to take care of their feet, and to educate them about the problems they can encounter and the treatment that is available for these problems (Haspel). Problems in the feet of patients with diabetes need to be detected early and treated appropriately or they may become gangrenous and lead to amputation of the toes or feet. Many treatments are available and there are many products on the market which can assist patients with diabetes in the care of their feet, and they need to be brought to the attention of such patients. Only with thorough, ongoing education and attention can patients with diabetes avoid devastating problems with their feet.

Center for Primary Care clinical guidelines for Type 2 diabetes and the National Institute for Clinical Excellence (NICE) 2003 technology appraisal of patient education models for diabetes state that effective care involves a partnership between patients and professionals, and all decision making should be shared (Clinical). referred to a multidisciplinary foot care team if they have new ulceration, new swelling, or new discoloration (redder, bluer, paler, blacker, over part or all of the foot) of the foot. Not all those with diabetes qualify for foot care coverage by Medicare (Goldsmith). In the mid-to-late 1990s Medicare changed coverage guidelines to exclude "healthy" diabetics from qualified routine foot care benefits. Patients with diabetes must additionally have related secondary complications of the lower extremities such as peripheral circulatory disorders and/or neurological manifestations in order to qualify for coverage. The ideal treatment of foot ulcers in diabetics consists of debridement, off-loading, infection control, and optimization of vascularity (Snyders). A multidisciplinary team is necessary for this, and part of that team is a specialist in orthotic off-loading devices. Several off-loading devices are available to aid in diabetic wound healing of the feet, including: standard below knee casts; prefabricated walkers; Charcot resistant orthotic walkers (CROW); The Integrated Prosthetic and Orthotic System; Healing Sandals, Darco's Wound Care System Shoe, and the Bledsoe Boot.
