MENTAL HEALTH AND MULTI-AGENCY TREATMENT AND CARE PLANS

The purpose of this paper is to formulate an assessment and care planning model for a case study involving an elderly female client. The paper specifies appropriate assessment and care planning frameworks, emphasizing the multidisciplinary, multi-agency, person-centered approach. Following the presentation of client information and the discussion of proposed assessment and care planning frameworks and approaches, conclusions are formulated based on the reviewed information.

Mrs Jones is an older woman who lives alone is partially sighted and has been diagnosed with leukemia. Over the last year, she has become forgetful and appears depressed. Not long ago, she was found by a neighbor who observed her throwing tablets of medication around the garden. Mrs. Jones has been seen by a General Practitioner, but the agency is unaware of the outcome. There are, however, signs of self-neglect in terms of her personal care. Also, she is not eating properly. In addition, Mrs. Jones constantly phones her neighbors asking them to come over and help her out, or just spend time with her. One neighbor, Mrs. Trigg, stated that she visits Mrs. Jones several times daily and yesterday she was there for four hours, as Mrs. Jones did not want her to go. Mrs. Jones also relies on other elderly neighbors to shop for her as she is finding it difficult to shop for herself.

The authors also note that the test has been subjected to comprehensive psychometric testing and shown to have good validity and reliability. While some of these tests, do assess for depression, it might be wise to include at least one additional standardized test for depression. Here, a good assessment instrument might be the Psychogeriatric Assessment Scales (PAS). According to Mackinnon and Mulligan (2001), the PAS has been widely used in European populations and maintains strong psychometric properties in terms of testing for both cognitive impairment and depression in older communities. In addition, there should be medical laboratory testing, imaging and specifically neuroimaging tests conduced as well as a complete neurophyschological evaluation to assess cognitive impairment. In addition, Mrs. Jones psychosocial circumstances must be taken into account as possibly responsible for at least part of her condition.
