DISCHARGE PLANNING FOR HOSPITALIZED ELDERLY: A PILOT STUDYCRITIQUE OF THE LITERATURE REVIEW, THEORETICAL

Naylor (1990, p. 156) adequately described the situation characterizing the discharge of many hospitalized elderly as a process that leads to undesirably high levels of rehospitalization, intensified physical and emotional stress for both patients and caregivers, and increased care costs. Naylor (1990, p. 156) cited quantitative data to support the assertions made in the review of literature.

Naylor (1990, p. 156) also presented information in the review of literature suggesting that a major cause of the undesirable outcomes associated with the discharge of so many of the hospitalized elderly lies in the low priority placed on discharge planning by most health care professionals. Data were presented indicating that such discharge planning is often absent even in the case of highrisk patients who clearly meet hospital guidelines indicating that such planning should occur.

The literature review develops a logical argument for the existence of a situation that supports to the stated purpose of the pilot study reported in the article. The stated purpose of the article was to "examine the effects of a comprehensive discharge planning protocol implemented by a gerontological nurse specialist as compared to the hospital's general discharge planning procedure" (Naylor, 1990, p. 156).

Naylor (1990, pp. 155156) discussed several "approaches designed to improve discharge planning and enhance postdischarge outcomes for the elderly" that have been tested. Naylor (1990, p. 157) observed that the tests of these approaches found that effective discharge planning for the elderly led to "earlier hospital discharge, reduced rates of hospitalization, decreased postdischarge complications, and health care savings."
