DATA COLLECTION QUESTIONNAIRE

The questionnaire included in this appendix is a cope of the instrument administered to the research sample for this study. Responses to the questionnaire were reported in Chapter 4 of the study.

This questionnaire is being administered as a part of a research study examining the effects of a stress management intervention program for managerial personnel in private sector organizations in the Republic of China. Specifically, the study is concerned with jobrelated stress levels among Taiwanese business managers. The questionnaire will be administered at two separate time points, to permit an assessment of the effects of the stress management intervention program. You will be asked to provide information about yourself, your job, and your organization. You will also be asked to provide information concerning events occurring in your life. All of this information will aid the assessment of the effectiveness of the stress management intervention program. You will not be asked to provide your name; therefore, the confidentiality of your responses will be safeguarded. Each of the questionnaire items dealing with jobrelated stress has two parts. The first part of each item asks that you state whether you have experienced this stress factor during the 30day time period preceding the day on which you completed the questionnaire.
