The Laparoscopic Procedure

A. The researchers in this study are looking to see if there is a difference between the early postoperative outcomes of laparoscopic gastric bypass surgery and open gastric bypass surgery. The problem is stated clearly. The problem is important to nursing in that the laparoscopic procedure resulted in shorter hospital stays and in terms of the types of complications seen after the two types of surgeries. The problem was a very simplistic one, which merely involved assessing the early outcomes of the two types of procedures. and was very simple to study because all it involved was determining the number and type of complications suffered by each group of patients, their length of hospital stay, time to return to normal function, and outcome in terms of weight loss produced.

B. The stated purpose of the study was to evaluate the early postoperative outcomes of laparoscopic gastric bypass surgery with open gastric bypass surgery using a matched pair analysis. The purpose of the study was clearly stated. Very little background is given regarding the study except to say that there are very few scientifically controlled studies in the literature, and that with the widespread dissemination of information over the internet, it would be very difficult to get patients to volunteer to be randomly assigned to open surgery for this procedure when it can be performed laparoscopically, and now that laparoscopic surgery is the treatment of choice for morbid obesity.

It is unethical to perform sham operations as controls, or to carry out the procedure on non-obese individuals as controls. There can be no control group in this type of study. It is simply a comparison between two techniques.
