The reduction of LOS, while maintaining the quality of patient care is, therefore, a high priority for hospital and clinic management. The role of physical therapy in this new, cost-centered health care environment will be determined in part by the ability of physical therapists to help the hospital and/or clinic operate efficiently within the constraints of the DRG reimbursement system. Should the operation of the physical therapy department contribute to a shorter LOS, or reduce overall costs in other ways, the hospital's financial goals will be positively affected.

Rapoport and Judd-Van Erd (1989) conducted a study to compare the length of hospital stay of a group of patients (N = 137) who were treated in a community hospital during a period of five-day-a-week physical therapy coverage with the length of stay of a similar patient group (N = 136) treated during a period of a seven-day-a-week physical therapy coverage. The comparisons of length of stay were made between the two groups, as well as between subgroups, based on diagnosis (stroke or orthopedic disorder) and surgical versus nonsurgical status.

The findings of this study support and underscore the importance of the provision of physical therapy. As shown in Table 1, for every subgroup except those patients with nonsurgical orthopedic diagnoses, the mean LOS was shorter for those patients in Group 2 than in Group 1. In only one subgroup, those patients with stroke who did not have surgery, was the difference in LOS large enough to be statistically significant at the 5% level. In this particular subgroup, LOS was about seven days (40%) shorter for those patients in Group 2 than in Group 1.
