2A. A survey instrument must appear neat and clear, it should not use a font that is too small or too faint. There should be plenty of space between sections and the entire thing should look open and not cluttered. It should be printed on a heavy stock of paper, especially if it is more than one page. If it is a person to person or telephone survey, there should be plenty of white space for the person giving the survey to take notes. If mailed, a survey should always include a stamped, self-addressed envelope to facilitate return of the instruments.

2B. Close ended questions are used when you want a specific answer to a question. This occurs when you want your respondents to choose, for example "Good" or "Bad," with no room for a third
