Due to the fact that the United States is projected to have a shortage of primary care professionals well into the 21st century, there has been an increasing use of nurse practitioners (Gamliel, Politzer, Rivo, Mullan, 1995, 133). This increase has led to the opportunity of establishing a business that at the same time can be a solution to the lack of access to primary care services.

If nurse practitioners were used to their full potential, estimates suggest that they and other advanced practice nurses could save the United States 20% of primary health care costs or between $6.4 and $8.75 billion annually. The quality of nurse practitioner services has been documented to be comparable to the quality of physician care in the primary care setting (Anderson, Gillis, Yoder, 1996, 210).

With the addition of skills in prevention, access to community-based resources, health education, and counseling. The combination of lower cost of nurse practitioner education, the effectiveness and quality of their services, and the lower cost of employment provide compelling reasons for the full use of nurse practitioners in primary care nursing (Gamliel, Politzer, Rivo, Mullan, 1995, 138).

Over the last 20 years, New York State has taken the lead in developing a range of home and community-based services for the frail elderly and disabled (Bunis, 1995, A17). A major goal was to avoid more expensive (and restrictive) care in a nursing home or hospital. Several different programs were developed, so that there would be a "continuum" of progressively cheaper and less restrictive alternatives -- from help with chores (which is often the only thing keeping a frail elder out of a nursing home or hospital) to using nursing-home services on a purely out-patient basis (adult day care).
