d to participate in relaxation training, cognitive restructuring activities, and so forth; she will sign a contract with the therapist agreeing to participate in these sessions and in the other interventions listed above. Mrs. Lopez and the primary caregiver will develop a listing of outcomes and measures that are to be used by both in evaluating the outcome of the interventions. Further, Mrs. Lopez will be asked to complete the Beck Depression Inventory on a scheduled basis during the intervention to monitor progress.
4) Finally, in conjunction with the advice of the facility psychiatrist, Mrs. Lopez will be maintained on her present medication regime until such time as the consulting psychiatrist and the patient agree that the medication can be changed, eliminated or otherwise refined and modified.
