To achieve the stipulated objectives, Mrs. Jones was briefly placed as an inpatient in a hospital that was part of a multi-agency and multidisciplinary program. During this time, social services, medical staff and medical specialist from diverse medical disciplines worked together to draw up a treatment care plan that would operate to assist with her vision problems, assist with her cognitive impairment, assist with her depression, and provide a variety of supportive social services. Also, Mrs. Jones was provided with psychiatric services to help her understand and make the transition to the new supportive environment.
