One of the key services provided by the PHS is that of medical research. This service is the domain of the National Institutes of Health. According to the Office of the Actuary of the Health Financing Administration (1991), in 1990, the PHS spent 12.4 billion dollars on medical research with most of these monies being funnelled through the NIH. Of this amount, only 1.5 billion was spent at the level of state or local government, with most of the monies being spent through the NIH's 12 constituent research institutions (e.g. National Eye Institute, National Cancer Institute, etc.)

Raffell and Raffell (1994) have noted that the NIH funds over 57 percent of all medical research conducted in the United States as part of its effort to meet its primary mission.
