Continuity of site refers to a patient having constant access to the same source of medical care such as a clinic or particular doctor (Continuity, 2000). In this sense, long-term care literature often uses continuity of care as a synonym for continuum of care, which has been described as "a client-oriented system composed of both services and integrating mechanisms that guides and tracks patients over time through a comprehensive array of health, mental health, and social services spanning all levels of intensity of care."
