As to the kinds of personnel which typically comprise public health boards at the state, local, regional and municipal level, Raffel and Raffel (1994) report that physicians have long dominated public health boards at both the state and the local level but that reform in the health care system is challenging their dominance on several grounds.

First, although physicians have always, as part of their practice, engaged in some preventative care, the great bulk of their professional work is remediative rather than preventative. Second, and related to the first reason for challenging the dominance of physicians on public health boards, is the fact that most schooling received by physicians is also remediative. Thus, they have little real training in preventative medicine.
