Mood: The patient did not display any depressed or anxious state. He acted as if he felt no extreme emotions.
Affect: The patient displayed appropriate emotions with limited range of affect and no extreme affects.
Thought process: The patient presented with logical and organized thoughts as he answered questions.
Thought content: The patient presented with odd thought content reflecting delusions.
Perceptual disturbances: The patient presented with auditory disturbances.
Cognitive: The patient presented with normal levels of alertness and orientation since he addressed questions, but his thought content was delusional.
Insight: The patient heard voices and did not mention that this was due to illness.
Judgment/Impulse control: The patient displayed impulse control with no outbursts, but did not present with judgment since he reacted to what he heard in his head, with arm movements showing where the voices came from.
