The following presents a discussion of an observational instrumental evaluation of videofluoroscopy (recording of images on a fluoroscopic screen on videotape) via modified barium swallow (MBS) and videoendoscopy (video camera in tip of endoscope) via flexible endoscopic evaluation of swallowing (FEESST). MBS and FEESST are described and the advantages of FEESST over MBS are noted.

Modified barium swallow (MBS) refers to the use of an x-ray that allows the radiologist to identify problems. This x-ray also allows the Speech Language Pathologist to determine why there are problems with swallowing. The MBS helps with the

diagnosis and treatment of swallowing disorders and communication problems. The MBS checks the mouth,

throat, and esophagus (Voice and Swallowing, 2011). Alternatively, the Flexible Endoscopic Evaluation of Swallowing with Sensory Testing (FEESST) does not include the use of an X-ray or barium administration. With the FEESST a radiologist or a radiology technician is not involved in the assessment. FEESST involves an endoscopy and provides information about how secretions are handled. FEESST also provides an assessment of hypopharyngeal sensitivity. These findings help the doctor determine issues related to swallowing and patient ability to protect their airway while food is ingested. Both the MBS and the FEESST require the participation of a speech language pathologist to help patients with management of swallowing problems. The speech language pathologist helps the patient understand related dietary and behavioral management issues. Benefits of FEESST include that immediate results are available and this procedure can take place in an office on an outpatient basis. The FEESST is non-radioactive and provides an alternative to MBS. FEESST is more portable and cost effective (Voice and Swallowing, 2011).
