Differential diagnosis for ODD must consider CD criteria and oppositional behaviors must not be found in association with Mood Disorders or Psychotic Disorders. ODD must also be distinguished from ADHD or mental retardation (if both are present, diagnosis for both must be made). The oppositional behaviors must exceed those commonly observed by the mentally retarded patient to warrant an ODD diagnosis. Behaviors resulting from impaired language comprehension or developmental stages do not warrant an ODD diagnosis.

ODD differs from CD in that the behaviors are not as severe and do not include serious physical aggression as found in ODD. ODD behaviors also differ from those found in CD in that they tend not to include aggression toward animals or people, destruction of property, or theft or deceit.
